
Allergies & Unique Mealtime Needs
DPS School Nutrition Services is committed to providing delicious, quality meals to ALL students.
In order to accommodate specific nutritional needs such as but not limited to: food allergies, intolerance or medical conditions, please submit the Unique Meal Request Form at the beginning of each school year, preferably 2 weeks before school starts to allow time to review and accommodate your student's needs.
Unique Meal Request Forms can be completed anytime during the school year as needed.

How to Complete the Form:

Parents/guardians fill out part A.
A licensed medical provider must fill out Part B.
Once complete SUBMIT via email to james_keaten@dpsnc.net with subject line "Unique meal request form for [insert student and school name]"
Notify your school nurse that a unique meal request form has been submitted
The form will be reviewed by School Nutrition, and the family will be contacted within 2 business days to discuss and finalize meal accommodations for the student. If you have not received a response within 2 business days, please contact our central nutrition office at 919-560-2370.
Please note: meal accommodations cannot be made for personal, religious, or cultural preferences.
Milk Substitution Request
If your child cannot drink cow's milk, DPS offers alternatives of Soy or lactose free milk. If this is your child's only dietary restriction, you can fill out a milk substitution form. This form can be completed by parent/guardian and does not require a medical provider input. A form must be completed each school year.
