CONSENT FOR RETESTING
Beginning with the 2025–26 school year, students who scored "not proficient" on their end of grade (EOG) test may participate in a second retest. This retest will give students the opportunity to improve their test scores. However, parental/legal guardian permission is required for a student to participate in a retest.

Please complete this form giving consent for your child to retake the EOG if they do not pass the first time. This is only for the EOG at the end of the year. Please provide your approval if you wish for your child to retake the test if they should not pass  https://shorturl.at/kPwA6
