• Open Enrollment for benefits for the 2022 calendar year for both the State Health Plan and Voluntary Benefits from Mark III (vision, dental, etc.) is:

     October 11 - October 29, 2021.  

     

     Make sure you enroll and log in to both platforms (State Health Plan and Mark III).

     OPEN ENROLLMENT INSTRUCTIONS:

    All active permanent eligible employees must complete their open enrollment for the 2022 benefit year.

    New employees who recently completed their initial enrollment MUST re-enroll during open enrollment for the 2022 benefit year.

    You have 2 enrollment options:

    Your user ID to enroll in health insurance is:

     

    • Login ID: Your first name, the first initial of your last name and the last 4 digits of your SSN.

     

    • If you transferred from another agency/district and already had an account in eBenefits, please use your first name, the first 2 initials of your last name, and the last 4 digits of your SSN.

     

    • Do not forget your tobacco attestation!

    Enroll over the phone by calling the Eligibility and Enrollment Center at 855-859-0966.

     

    Extended hours during Open Enrollment:

    ·       Monday-Friday, 8 a.m.-10 p.m.

    ·       Saturdays, 8 a.m.-5 p.m.

     

     

    Forgot Your Password for State Health Plan's eBenefits?

    You can reset your password yourself and DO NOT need to call the Eligibility and Enrollment Support Center to reset your password.

    If you are having issues logging into eBenefits, do not continue to attempt to log in or you will lock your account. Instead you have the option to reset your password. Simply click “Reset your account” on the log-in page, and then click “I can’t remember my password." From there, you will be prompted to a screen that will ask you to enter your username, so a passcode can be sent to the email address you have in eBenefits.

     

     Dental, Vision, Life, Disability, etc.

    Employees on a leave of absence will not be able to enroll for voluntary benefits beginning January 1, 2022 until back at work.

    Your user ID to enroll in Mark III benefits is:

     

    • Login ID: Your Full SSN (no dashes).

     

    • Password: Last 4 digits of your SSN, followed by the last 2 digits of your birth year.

     

    • To sign your elections, use your PIN: Last 4 digits of your SSN, followed by the last 2 digits of your birth year.

    Forgot Your Password for Mark III?
