REGISTRATION FOR COVID-19 STAFF TESTING

  • DPS Staff Members may register for weekly COVID-19 surveillance testing by locating your school or common employment location and clicking the link underneath. For staff members that have multiple locations, please choose the individual form for the location you will be present at during that location's testing window or the Central Services location at the SDC. For example, a teacher that teaches at multiple schools would choose the school where they are present when COVID testing is taking place OR could sign up for the Central Services location at the SDC.
