New health assessment requirements for children new to public schools
In 2015 the North Carolina legislature passed House Bill 13 which states that a health assessment is now required for all children enrolling for the first time in a North Carolina public school in grades K-12. House Bill 13 is now in effect for 2016-17 for all K-12 students coming to North Carolina from another state or country and those enrolling for the first time coming from a home-school, religious or private/independent school.
The assessment must occur within 12 months prior to entering school. The medical provider, parent or guardian must provide a completed health assessment form to the school on or before the child's first day of attendance. Health assessment forms are also available here in English and Spanish, at all Durham Public schools, area doctors' offices or at the Durham County Health Department.
