State Health Plan Annual Enrollment 2008
March 1 to March 28, 2008 (Coverage changes will be effective July 1, 2008)
The Indemnity plan is no longer being offered as an option through the State Health Plan. During this enrollment period, the State Health Plan is pleased to offer you a choice of three PPO plans: PPO Basic, PPO Standard or PPO Plus. As an Indemnity plan member, you must enroll in one of these plans. If you do not select one of the PPO plans, you will automatically be moved to the PPO Standard plan.
More information about all three of the PPO plans, as well as other materials that will help you select the PPO plan that is right for you and your family can be located on the State Health Plan website http://statehealthplan.state.nc.us/ . PPO members do not need to do anything unless you need to make changes to your plan selection or to add/delete dependents.
