Transfer Request Forms
DPS will respond to your request within seven business days from receipt of your application with written notice regarding your child's school assignment and transportation availability. You will then have an opportunity to communicate whether you accept the approved transfer or want your child to remain at his/her current school. If a transfer request is approved, your child will be allowed to attend the transfer school through the fifth grade.
Please note: If your child’s base school’s AYP status changes in subsequent years and is no longer is required to offer the Choice requirement under No Child Left Behind law, your child may remain at the transfer school, but transportation will not provided. If the requests for transfer exceed available funding, priority must be given to the lowest-achieving students from low-income families.
Schools that participate in the Title I program receive funds from the federal government that are used to help students who are not achieving at grade level in low-income schools. As a recipient of these funds, Title I schools are required to make AYP in improving student achievement or face increasingly serious consequences.
