Tdap Vaccine Required for Rising 6th Graders

Notice to Parents/Guardians of Rising 6th Grade Students

Students entering the 6th grade during the 2011-2012 school year, and attending public schools are required to receive a booster dose of Tdap (tetanus, diphtheria and pertussis) vaccine before the start of school.

Parents/Guardians will be asked to provide proof of immunization (i.e. Certificate of Immunization) on the first day of school which is July 18, 2011 for year-round schools and August 25, 2011 for traditional schools.

The Durham County Health Department will provide the Tdap vaccine free of charge while supplies last at:

Durham Public Schools Staff Development Center
2107 Hillandale Road
Durham, NC 27705
Dates:  May 3, 2011 and May 24, 2011
Time:   3:00pm-6:00pm

Please bring your child’s current immunization records!
